Gaslighting in Rheumatology: Why "Normal" Treatment Isn't Always Good Enough

If you’ve recently been diagnosed with Rheumatoid Arthritis (RA) or another inflammatory joint condition, you likely feel a mix of relief and fear. Relief that your pain finally has a name, but fear of what comes next.
Unfortunately, many patients find themselves trapped in a medical "waiting room" even after their diagnosis. They are put on medications that make them sick, kept on steroids with no exit plan, and told that their ongoing pain is just something they have to live with.

The Cost of the "Three-Month Wait"
In rheumatology, there is a "window of opportunity." The earlier you treat RA effectively, the better your long-term outcomes. Early, aggressive treatment leads to:

Safety & Efficacy Check: Your rheumatologist should assess you at the one-month mark, not the three-month mark.
The "Bridge" Strategy: If you can't tolerate the initial drug, your doctor can work with drug companies to provide "bridge" samples of biologics while waiting for insurance paperwork to clear.
No "Marriage" to Meds: As Dr. Amigues says, "We do not marry medications; we date them." If a drug has a "red flag" side effect, it’s time to move on.
Defining True Remission: The "Rule of Five"
Many patients think they are in remission because they "feel a bit better." But clinical remission is a specific target. To be in Minimal Disease Activity, you should meet at least four of these five criteria:
No more joint pain.
No more joint swelling.
Morning stiffness lasting less than 15 minutes.
No systemic fatigue.
Normal inflammatory markers (CRP/ESR) in your blood work.
If you are still experiencing these symptoms, or if you are only feeling better because you are taking Prednisone (steroids), you are not in true remission.
The Danger of the "Forever Steroid"
Steroids are a miracle for putting out the fire of a flare, but they are not a long-term solution. Being on Prednisone for a year or more carries heavy risks:
Adrenal Insufficiency: Your body stops making its own natural steroids.
Osteoporosis: Bone thinning and increased fracture risk.
Metabolic Issues: Weight gain, hypertension, and diabetes.
Every steroid prescription must have an exit plan. If your doctor hasn't discussed how and when you will taper off, your long-term health is at risk.
